Toksik guatr’ların tedavisinde; başlangıçta ilaçlar kullanılabilir. Belirli bir süre bu tedavi devam edebilir. Toksik guatr’ın tipine göre daha sonra radyoaktif iyot veya cerrahi tedaviye karar verilebilir. Bu karar; hastanın tüm tetkikleri tamamlanıp doktorlar tarafından belirli bir süre takip edildikten sonra verilir.
Toksik guatr’lara hangi tedavi seçeneğinin uygulanacağına hangi kriterlere göre karar verilir?
Nodüler veya multinodüler olanlarda nodül veya nodüllerin iyi veya kötü huylu olup olmadıkları ortaya konmalıdır. Bunun için hastanın şikayeti, hikayesi, muayene bulguları, ince iğne sitoloji bulguları gibi verileri kullanarak bir karara varılır. Sonuçta; iyi huylu toksik nodüler guatr tanısı konursa, radyoaktif iyot ile tedavi veya cerrahi tedavi tercih edilebilir. Kötü huylu toksik nodüler guatr tanısı konursa ise mutlaka cerrahi tedavi uygulanır.
Toksik difüz guatr’lar yani diğer ismi ile Basedow-Graves Hastalığı olanlarda tedavi seçenekleri genellikle radyoaktif iyot veya cerrahidir. Özellikle oftalmopati belirgin ise cerrahi tedavi tercih edilir.
